Earthworm Jim

After you beat the last boss, it says, "U're the best" Then, Jim walks outside and sees the princess. His face turns into that of a wolf's and the princess blows a kiss at him. Then, a cow falls on top of the princess (Jim's jaw drops down and the princess's crown is hurled to the ground) and he mopes away. The ledge breaks and the princess and cow fall into the lava. Earthworm Jim peeks out from the edge of the screen and he grabs the crown. He runs away and then the credits roll. At the very end, it shows a greyscale picture the creators of Earthworm Jim and Jim himself.
